What is an Ontario Health Card?

When you apply to the OHIP and you approved, you get an Ontario Health Card that proves that you covered by OHIP so the insurance will pay for the health services you may need, like doctor visits, medical tests or surgeries.
When your health card expires, you will need to renew it if you want to keep having the coverage of OHIP.
How do I do the renewal of my Ontario Health Card?
The renewal of your Ontario Health Card can done online or in a Service Ontario center.

The requirements you must have to renew your health card online are your current’s driver’s licence, your most recent Health Card, a valid credit card (Visa or MasterCard), and a printer so you can print your temporary documents. In case you have everything, you just have to fill out an application online and pay. Once you done, you will have to download and print the documents the website will give you. You must carry those temporary documents until your new health card arrives. It usually takes between 4 and 6 weeks.
If you prefer to go to a Service Ontario centre, you will have to bring the following: your current health card, the reminder that was sent to you by mail a couple months before your health card expired, your renewal reminder with the completed Health Card Renewal Form, documents that prove your personal identity and Ontario residency, your most recent citizenship or immigration document, and your marriage certificate in case you are going to apply under your marriage name. In this case, your new health care will also mail to you and it will take between 4 and 6 weeks to arrive.
How do I replace my Ontario Health Card?
If you Lost Health Card Ontario or damaged, stolen, you must go to a Service Ontario centre to replace it.

If you want to replace a red and white health card, you must bring three different original documents that prove citizenship or immigration status, residency in Ontario and identity.
If you are replacing an expired health card you must bring original documents that prove your residency in Ontario and your identity. The same applies if it is an unexpired health card and your address or name changed. If it is an unexpired health card and your address or name have not changed.
